Recovery Specialist - MSW
MetroClub PROS
MetrocClub PROS (Personalized Recovery Oriented Services) provide safe, nurturing, non-institutional settings for psychiatric rehabilitation that foster independent living and greater participation in the community. Clubhouse members play an active role in their own rehabilitation as they participate in structured activities and groups as well as the day-to-day operation of the Clubhouse to help them explore and obtain employment, housing, education and a more active social life in the community.
Responsibilities:
- Manage a caseload of approximately 20 consumers, including case management such as collateral contacts, referrals, vocational assistance, etc, to coordinate services.
- Conduct required assessments including Wellness Recovery Action Plans, Psychosocials, Psychiatric Rehabilitation, Vocational Readiness, Cultural, and Chemical Abuse or Dependence Screenings.
- Develop Individualized Recovery Plans (IRP) in collaboration with the recipients, based on the above listed assessments on a semi-annual basis, or more often as needed.
- Develop and facilitate approximately 10-14 groups, according to protocol, which will cover CRS, ORS, IR and Clinical Services as dictated by the IRPs of the program participants.
- Conduct individual counseling sessions to assist program participants with attainment of the life-role goals specified in their IRPs.
- Document consumers' progress/regression towards goals in monthly progress notes and determine the need for addition or subtraction of services offered to each participant on an ongoing basis.
- Coordinate treatment including the sharing of Individual Recovery Plans and Progress Notes for participants who are co-enrolled with other agencies.
- Provide services according to Evidence Based Practices including Wellness Self Management, Family Psychoeducation, Medication Management, Co-occurring Disorders, and Supported Employment.
- Coordinate follow-up services once a participant is ready for discharge from the program.
- Act as a member of an interdisciplinary treatment team, including participation in weekly administrative, clinical and utilization review meetings.
- Overall responsibility for maintenance of client records, including timely documentation of services as required by Medicaid and the Office of Mental Health.
- Develop and maintain collateral relationships with consumers' other providers and family as clinically indicated
- Carry a 24 hour support cell phone in rotation with the clinical team.

MSW (LMSW preferred) required. Previous experience working with adult mentally ill/MICA population preferred. Knowledge of running groups and required. Computer skills and knowledge of service documentation required. New graduates with internship/volunteer experience welcome to apply. Social Worker, East New York /Bedford Stuyvesant Family Centers
Our preventive services focus on ensuring child safety; helping children develop and reach their full potential; and strengthening families. BCS has been a nation-wide leader in family preservation services. Our prevention program utilizes Structural and Strategic Family Therapy (SFT), a clinical approach that is goal oriented and outcome focused. Using this approach, BCS maintains a staff of Master's level Social Workers who are trained to assist and strengthen families in their ability to manage the complex conditions they face.
RESPONSIBILITIES:
- Provide intensive weekly family/individual counseling to a small caseload families
- Develop comprehensive treatment plans along with the appropriate case planning and assessment
- Maintain timely case notes

Residential Habilitation Trainer (per diem)
Residential Habilitation teaches independent living skills to developmentally disabled adults living with their families or in their own homes.
Responsibilities:
- Provide hands on training to assigned consumer(s)
- Maintain documentation

Individual Residental Supportive Housing

Our Supportive Housing Program, located in Bedford Stuyvesant, provides 12 studio apartments to adults with developmental disabilities and helps residents live a full and integrated life in the community. Staff assist residents cultivate the skills needed for independent living.
Responsibilities:
- Provide individual instruction and planning as per individual needs; follow and implement Habilitation Plans
- Escort individuals to and from medical appointments
- Write daily notes for individuals that meet OMRDD and Medicaid guidelines
Requirements:
BA required; Human Services field preferred. Excellent interpersonal and verbal communication skills are required. AMAP (Approved Medication Administration Personnel Certificate required); CPR; and First Aid Training are required. NY State valid driver's license preferred. The Atlantic Avenue Early Learning Center serves infants, toddlers, preschoolers and pregnant women through a broad range of early learning services fostering children's growth and development while helping parents build strong parenting skills. Our Early Childhood education program is designed to enhance the physical, social, emotional, cognitive, and language development of young children through high quality educational and social services for themselves, their families, and for pregnant women.

Recovery Specialist - Kitchen and Maintenance

Responsibilities:
- Foster an atmosphere of congeniality that facilitates the implementation of the basic philosophy of Metro Club PROS.
- Manage a caseload of approximately 15 members, including case management such as collateral contacts, referrals, and vocational assistance to coordinate services.
- Document services in a comprehensive timely manner as indicated by PROS requirements.
- Conduct required assessments including Psychosocials, Psychiatric Rehabilitation, Vocational Readiness, Cultural, and Chemical Abuse or Dependence Screenings (within 30 days)
- Develop Individualized Recovery Plans and Wellness Recovery Action Plans in collaboration with the recipients, within the first 30 days of participation, based on the above listed assessments. Review with participants every 3 or 6 months, or as needed. Provide intensive services as needed.
- Develop and facilitate approximately 10-14 groups per week, according to protocol, which will cover CRS Services as dictated by the IRPs of the program participants.
- Conduct and document individual counseling sessions weekly to assist program participants with attainment of the life-role goals specified in their IRPs.

Kitchen/Maintenance
- Perform and oversee all food service functions including nutritious and timely meal planning and production that meets DOH and CACFP regulations.
- Ensure successful meeting, documentation and audits of DOH and CACFP regulations.
- Order food and supplies; provide ongoing inventory. Reconcile Food and Maintenance related funds.
- Liaise with ENYCH staff regarding work and payment for Maintenance Transitional Employment position.
- Act as liaison with accounts regarding food and maintenance related funds, CACFP billing, payment of maintenance Transitional Employment trainee.
- CACFP Oversight of policy, procedure and submitting monthly meal count reports for billing submission & reimbursement.
- Actively participate in and assume bottom line responsibility for the maintenance of Metro Club PROS assuring cleanliness in the kitchen and common spaces.
- Ensure that evening, weekend and holiday meals are adequately planned and executed.
- Communicate with Metro Club Community and BCS maintenance personnel to assure the completion of larger maintenance tasks.

Trainer, Day Habilitation
Day Habilitation helps individuals with developmental disabilities cultivate the skills required for independent living including navigating public transportation, providing personal financial management training, and offering opportunities for socialization and volunteering in the community. Day Habilitation offers clients vocational counseling and training to help them participate more fully in community life.
Responsibilities:
- Provide habilitation services to individuals with developmental disabilities.
- Provide consumers with travel training and safety training in the community.
- Facilitate groups.
- Provide supervision at the volunteer site.
- Document face-to-face daily services and monthly summaries.
- Follow program and Medicaid documentation guidelines.
- Develop and review the Habilitation Plan with consumer, MSC, family/advocate and other service providers.
- Ensure required documentation is on file.
- Help consumers enhance their literacy skills by using the Reading Horizons program.

Medicaid Service Coordinator
BCS's Medicaid Service Coordination provides assistance to people with developmental disabilities in gaining access to necessary services and supports appropriate to their needs and life goals. It is provided by qualified service coordinators and uses a person centered process to develop, implement and maintain an Individualized Service Plan (ISP).
Responsibilities:
- Develop, implement, maintain, review, and oversee the Service Plans for a caseload of MR/DD consumers. Maintain and oversee case records to meet audit standards.
- Liaise with other service providers to ensure coordination of services on behalf of consumers.
- Locate resources, programs, and other services for consumers. Assist consumers in maintaining Medicaid and eligibility for services.
- Conduct face-to-face meetings and other contacts with consumers, including visits to their homes. Hold family and advocacy meetings to ensure service satisfaction.
- Respond to on call emergencies.
- Conduct ARS intakes.
- Other responsibilities as needed.
Requirements:
Bachelor's degree in a health or human service field required. One year experience working with individuals with developmental disabilities or one year experience working as a service coordinator with any population required. With a Masters degree in a field of human services, the minimum work experience is not required. Good oral and written communication skills. Must be skilled in handling emergencies and urgent matters while staying organized. Must be flexible to do field work and be on call as needed. Bilingual (English/Spanish) preferred. Clinical Supervisor
Brooklyn High School for Leadership and Community Service
Brooklyn High School for Leadership and Community Service is a small, academically rigorous, full-time New York City Department of Education Transfer School designed to re-engage high school students who are over-age and under-credited or have dropped out of high school. BCS is directly responsible for the administration of the school's Learning-to-Work Program, which enhances the school's academic component by offering additional academic support, career and education exploration, work preparation, skills development, and internships.
Responsibilities:
- Responsible for Intake process/coordination throughout the year; organize orientation activities for students and families
- Provide direct individual counseling and group counseling to students who are referred by Advocate Counselors and teaching staff; provide crisis intervention as needed; refer students to academic, personal , and any services in community as needed
- Provide counseling and support services to families with the intent of improving communication within the families of students and ensure students and families have the support they need to function at optimal levels
- Supervise three Advocate Counselors; provide daily counseling support to Advocate Counselors via advisory planning and implementation, attendance/disciplinary protocol, and individual counseling strategies; lead and monitor Advocate Counselors in daily morning attendance check-in meetings; participate with Advocate Counselors with home visits as needed
- Develop a physical and mental health workshop series ; research/organize personal development trainings based on staff areas of need
- In collaboration with Program Director and Learning to Work Coordinator, organize and run community meetings with entire student body
- Participate in all Attendance Action Committees
Requirements:
Masters Degree in Social Work (MSW) required; LMSW preferred; SIFI certified a pus. Experience and demonstrated skill in working with youth and families required. Prior experience in a school setting preferred. Excellent organizational, written, and communication skills. Must have a commitment to work from a strength-based and/or youth development perspective. Work Readiness Counselor
Work Readiness Services prepare participants for competitive employment. Participants engage in work related activities to develop necessary skills to achieve and maintain employment. These activities may include skills training, computerized assisted learning, cognitive remediation, and work experiences in the community.
Responsibilities:
- Assist individuals with developmental disabilities develop work readiness skills necessary to secure and maintain community based employment.
- Carry a caseload of 18-22 consumers.
- Conduct and/or arrange work readiness assessments.
- Provide individual counseling and case management services.
- Run work readiness groups to include wellness groups.
- Arrange for skills training.
- Refer consumers to community-based training sites.
- Refer consumers for placement services.
- Complete monthly reports and other required documentation.
- Submit monthly reports to referral sources.
- Maintain ongoing contact with referral sources and other service providers.
- Prepare a referral packet for MSC services.
- Participate in vendor fairs and other program recruitment events as necessary.
- Complete intakes for the Work Readiness program and ARS department.
- Other responsibilities as needed.
Requirements:
BA degree in the Human Services related field required. Experience working with the developmentally disabled population required. Counseling skills required. Home Visitor (Bi-lingual English/Spanish)
Our Atlantic Avenue Early Learning Center serves infants, toddlers and pregnant women through our center and home-based services, provides a broad range of early learning services fostering children's growth and development while helping parents build strong parenting skills. Early Head Start services are designed to enhance the physical, social, emotional, cognitive, and language development of young children through high quality educational and social services for themselves, their families, and for pregnant women.
RESPONSIBILITIES:
Facilitate home visits aimed at helping at-risk expectant and new parents gain the skills and support they need to ensure the health and well-being of themselves and their children. Lead discussions and activities related to pregnancy, prenatal and postnatal care, and parenting skills. Use child assessment information and observations to guide parents in planning; implement home visit activities; and follow-up during home activities.
- Develop service plans with families and complete periodic child development assessments. Carry a caseload of twelve families.
- Conduct outreach to recruit families.
- Co-facilitate a variety of prenatal and parenting groups aimed at helping parents develop parenting skills and build stronger relationships with their children.
- Develop strong, positive working relationships with families; help families access needed resources and services to improve family functioning and well-being.
- Actively participate in team meetings and supervision sessions. Collaborate with small program team to implement a high-quality new program that delivers favorable outcomes.
REQUIREMENTS:
Child Development Associate (CDA) certificate plus two years experience promoting healthy growth and development with infants and toddlers including planning activities, observing children's behavior, and supporting and facilitating parents' learning of their infants/toddlers' development and behavior required. BA/BS or AA in Early Childhood Education or related field preferred, high school diploma with some early education college credits required. Bilingual English/Spanish required. Must be able to travel independently to the homes of participating families. Criminal background check and fingerprinting required.
